Change in Student Record Policy
Purpose
To facilitate effective communication with students and to ensure accuracy of student records.
Policy
The College expects students to report any name, address, intended program/major, telephone number or other record changes on the form available at www.psozxd.com/form.
Students who have name changes must provide the legal documentation as specified on the form available at www.psozxd.com/forms. Degrees are awarded under the name the student has on file at the time the degree requirements are completed.
Academic records are maintained under a student’s legal name at the time of enrollment. Academic records and credentials are not modified unless the student has an active registration with the College.
